IS A GROUP OF 501(c)(3) NON-PROFIT PROFESSIONALS, MEMBERS AND VOLUNTEERS WHO SUPPORT HOLISTIC WELL-BEING, HEALTH EQUITY, WHOLE-PERSON HEALTH AND CARE. WE LEVERAGE INFORMATION TECHNOLOGY, DIGITAL TOOLS, DIGITAL HEALTH-TECH, AI & ANALYTICS TO INSTITUTE INCLUSIVE SERVICES AND PROGRAMS THAT FURTHER

PURPOSE.
Overcoming disparities, barriers to holistic well-being and health equity in urban, semi-urban, and rural communities. CareHealthConnect extends special support to resource constrained, underserved, vulnerable individuals and communities.
Holistic well-being is many things at the sametime, and good health is an essential aspect of Quality of Life.
Holistic well-being depends on multiple essentials as listed under the seven important dimensions that come together to meet basic needs of whole-person health. Namely:
Good nutrition & protective clothing
Good health
Convenient housing
Financial stability
Learning & opportunities
Community & social connections
Environment & other
A short-coming in any of these dimensions could adversely impact an individual’s quality-of-life. The inter-connected nature of each dimension when not adequately met, puts extra strain on other dimensions. External care, support systems and systematic resource availability can alleviate some of this strain when properly planned and structured.
